The Two Minds You're Working With
You've experienced this even if you've never named it: you know the late-night snacking isn't serving you, and you do it anyway. You know the worry is disproportionate, and it worries you anyway. That gap — between what you know and what you do — is the gap between your conscious mind and your subconscious mind.
The conscious mind is logical, deliberate, and easily tired. The subconscious is the feeling mind: it stores your learned patterns, emotional associations, and the beliefs quietly running your life, and it automates your behavior from them. Willpower lives in the first system. Your habits live in the second. This is why change that only targets the conscious mind — information, advice, rules — so often fails to stick: it's aimed at the wrong system.
Hypnotherapy is simply a method for working with the second system directly. And the doorway to it is a change in brain state.
Brainwave States: Why Relaxation Is the Doorway
Your brain's electrical activity shifts with what you're doing. In ordinary alert, analytical mode, faster Beta rhythms dominate. As you relax deeply, the brain shifts toward slower Alpha rhythms — calm, absorbed focus — and, deeper still, toward Theta, the state associated with drifting toward sleep, vivid memory, imagery, and heightened receptivity to suggestion. Theta is prominent in young children — which is part of why childhood is when so many of our core beliefs were absorbed without question in the first place.
Hypnotic induction — the guided relaxation at the start of a session — is, in brain terms, a deliberate shift out of analytical Beta dominance and into these Alpha/Theta states. The analytical gatekeeper relaxes. The feeling mind becomes reachable. That's not mystical; it's the same continuum of states you pass through every night on your way to sleep. In a session, we simply hold the brain in the receptive part of the continuum, awake and aware, and use it on purpose.
What Brain Imaging Shows During Hypnosis
This is where the skeptics tend to sit up. Researchers at Stanford — led by Dr. David Spiegel, one of the most cited hypnosis researchers in the world — have used fMRI to scan people's brains during hypnosis. Their findings, published in Cerebral Cortex in 2017, showed distinct, measurable changes in three brain systems during hypnosis:
- The brain's alarm-and-salience system settles. Activity drops in the dorsal anterior cingulate cortex (ACC) — a region involved in scanning for problems and deciding what deserves worry. In hypnosis, the brain temporarily stops guarding the door.
- Mind-body connection strengthens. Connectivity increases between the prefrontal cortex and the insula — a pathway involved in how the brain controls and perceives what's happening in the body. This is one reason hypnosis can influence physical experiences like tension and pain.
- The self-critical narrator disengages. Connectivity decreases between the executive control network and the default mode network (DMN) — meaning actions and suggestions get processed with less interference from the brain's self-referential commentary.
In plain language: under hypnosis, the brain becomes less defended, more embodied, and less self-conscious. Which is precisely the state you'd want a mind in before asking it to reconsider a decades-old belief.
The Inner Critic Has an Address in the Brain
That default mode network deserves a closer look. The DMN is the network your brain runs when it isn't focused on a task — and it's where rumination lives: the replaying, the self-judgment, the "why did I say that," the story of who you are and what you're capable of. For many of my clients, the DMN is where their harshest inner voice holds office hours.
An overactive, negatively-toned DMN keeps old beliefs in heavy rotation. So a state that temporarily turns down DMN dominance — which is what the imaging shows hypnosis does — creates a rare window: the old story pauses, and a new one can be heard. This is the neuroscience behind something I see in almost every session: insights surface in hypnosis that clients could never reach by thinking harder.

Neuroplasticity: How New Beliefs Become New Wiring
None of this would matter if the changes couldn't last. They can — because of neuroplasticity, your brain's lifelong ability to rewire itself based on what it repeatedly experiences and rehearses. The principle, familiar to any neuroscience student: neurons that fire together, wire together.
Your current patterns are physically real — well-worn neural pathways, strengthened by years of repetition. That's why they feel automatic. But the same mechanism that built them can build their replacements. Repetition made the old belief strong; repetition makes the new one stronger. A 2019 systematic review and meta-analysis of 22 randomized controlled trials, published by the American Psychological Association, found self-hypnosis effective across pain, stress, anxiety, and childbirth, with a medium-to-large overall effect size — and zero adverse effects reported across all 22 trials. In weight loss, Kirsch, Montgomery & Sapirstein (1995) found adding hypnotherapy more than doubled results, holding at two-year follow-up.
